Welcome!

The fact that you have arrived here means that your child has been tested or will soon be teseted with Dr. Cates.  This site is designed to receive and retain securely teh information Dr. Cates will need before testing your child.

If you have more than one child, you will have the opportunity to register each child on this site.  You alone as the parent will enter data and control the account.  If you have any questions you may contact us at any time.

Below is the order you will fill out the forms.  If you have been through this sequence before, some forms may only need to be edited or updated by you, or skipped altogether.

Pre-Testing Sequence

1. Register each child that will eventually be tested.

2. Fill out or update the Case History Form.

3. Fill out the Tova Inventory.

4. Fill out the Curriculum Form.

Viewing and Editing Previous Entries

Each time you fill out a new form the entries are stored on the Tables page.  You will find a link to this and other pages you might need in the navigation bar at the top of the site.
